children and families make informed
decisions in regards to their family’s needs
by providing:
-
Outreach and Referral
-
Care Coordination
-
Case Management
-

-

-
Follow-up
Families who are concerned that their child
(ages 0-25) may have Mental Health Issues,
Behavioral Problems, Substance Abuse
History, Developmental Disabilities,
Interpersonal Violence Issues, other
concerns can call the Get Connected Family
Resource Center and our Family Navigators
will help you find the resources you need.
We have a large database of resources
including information on local
speech-language therapists, occupational
therapists, physical therapists, parent
support groups and information
organizations, child and adult mental health
providers, crisis services, psychologists
and psychiatrists, trauma services, shelter
services, local providers of clothing, food,
household goods and other auxiliary services
such as transportation, housing, clothing
etc. We often can tell you what insurances
they take, if their facility is handicap
accessible, if they have bilingual staff
etc.
Family Navigators are people who have had to
“navigate” the systems of care for their own
children. If they can’t provide you with the
resource you need over the phone they will
meet face to face with you and work with
them to prioritize your needs and construct
a Family Care Plan, utilizing your family’s
strengths and the Get Connected Database to
identify appropriate resources. Families
that have children with multiple
disabilities such as mental retardation,
blindness, orthopedic impairment, mental
health issues, substance abuse etc. are
encouraged to contact our center for help
locating appropriate resources.
All services are family driven; meaning that
we encourage families to prioritize their
own needs and choose the path of treatment
options they would like to take. Services
are available for families whose disabled
child is in transition between child and
adult service providers as well.
The Get Connected Family Resource Center is
open Monday through Saturday (see hours
posted under the contact us page) and is
located in the Carroll Nonprofit Building in
Westminster. |
